This agreement Is effective 12:01 AM local time on the effective date (date signed. and for the period indicated. Prices are subject to change after completion of the initial term indicated. Annual visits will be performed during normal business hours, and under the schedule or Milestone (ME). Annual visits will be performed upon your request and will not roll over after the first 12 months. Plans will auto-renew for monthly periods upon completion of the initial term.
Cancellations require a letter sent to ME at least 15 days before the next renewal date in order to avoid renewal charges. The letter must be received by ME within this timeframe.All monies paid are non-refundable. All promotions, offerings. discounts, and anything else stated are valid only while account is paid and in good standing. Agreements will become void with a past due account (over 1 day). I agree to term for at least 12 months. The visit/tune ups have no monetary value and are not a contractual component to this agreement.
(1) Extended Guarantee(s) ONLY covers the work previously performed by ME as a malfunction of its installation and or equipment. It does not cover normal wear and tear, previously existing conditions, or security equipment. (Dispatch fees and values vary.)
(2) Subscriber will pay an emergency fee for ELECTRICAL, HVAC & PLUMBING SERVICE work for as long as they are a plan member. Emergency service is described as loss of power to the home, AC unit, furnace, or any major appliance outside of normal business hours. Plumbing emergency service is described as busted pipes, water heater leaks & whole home drain back ups outside of normal business hours.
(3) All visits requested by ME must be completed in order to maintain guarantees.
(4) HVAC, Plumbing and Electrical visits are not a contractual component to this agreement. ME will perform visits as their schedule allows. or at the request of the customer. lune Ups / Visits nave no monetary value. and are not a contractual component to this agreement. All final requests for visits must be made in writing to milestone to 2360 Crist Rd. Ste B900 Garland TX 75040. All visits expire and terminate after agreement period is complete.
(5) All Guarantees Apply To Residential Only. Limit up to S20,000. Must be within (30) days of the completed services. All money back guarantee requests applying to MAM plan must be submitted in writing within 90 Days of start date.

Note: DISCLAIMER- Members must complete 12 months of Milestone Advantage membership to retain 15% members-only discount toward services. Membership cancellation prior to 12- month completion will result in a required reimbursement payment by the customer for the sum of all discounts applied toward services by Milestone Home Service Co., paid in full upon cancellation
